Fay was a Bath County native. She was a homemaker and member of the Church of God.
Fay is survived by her sister, Jeanette (Thomas) Anderson of Owingsville; one nephew, Paul (Danette) Anderson of Owingsville; great nephew, John Anderson and great niece, Madison Anderson.
Fay was preceded in death by her husband, Robert Cook; her parents, T.J. Hovermale and Naomi Reed; and one great nephew, Paul Travis Anderson.
